As we continue to expand our services, we are seeking a motivated and experienced Remote Family Nurse Practitioner (FNP) to provide telehealth services to patients across multiple states. Position Overview We are seeking a skilled and dynamic Family Nurse Practitioner to join our growing telehealth team. The ideal candidate will have experience in primary care, functional medicine, hormone optimization, wellness focused care, and chronic disease management. This role is fully remote and designed for a provider located within the Denver Metro Area who can occasionally participate in team meetings, training, or collaborative initiatives as needed. Candidates holding licensure in multiple states are strongly preferred, as we are actively expanding our nationwide telehealth footprint. The FNP will work closely with our existing medical team to provide comprehensive virtual care, interpret advanced laboratory testing, develop personalized treatment plans, and support patients through ongoing health optimization programs.

Key Responsibilities

• Conduct comprehensive telehealth evaluations, including medical history reviews and virtual assessments.
• Order, interpret, and manage laboratory testing, including functional and specialty testing.
• Develop individualized treatment plans focused on root-cause medicine and long-term health outcomes.
• Manage patients with chronic health concerns, metabolic conditions, hormone imbalances, and preventative health needs.
• Prescribe medications and supplements when clinically appropriate and permitted by state regulations.
• Provide patient education and ongoing support to improve treatment adherence and outcomes.
• Maintain accurate and timely medical documentation within the electronic medical record.
• Collaborate with other medical providers and clinical staff to ensure continuity of care.
• Monitor patient progress and adjust treatment plans based on clinical response and laboratory findings.
• Participate in team meetings, clinical training, and ongoing professional development.
• Support the growth and expansion of the clinic's telehealth services across multiple states.

Qualifications & Skills Required

• Active Family Nurse Practitioner (FNP) license.
• Minimum of 2 years of clinical experience as a Nurse Practitioner.
• Experience managing patients through telehealth platforms.
• Strong clinical decision making skills and ability to practice independently.
• Excellent communication and patient relationship building skills.
• Proficiency with electronic medical records and telehealth technology.

Preferred

• Licensure in multiple states.
• Functional Medicine training or experience.
• Experience in hormone replacement therapy (HRT), metabolic health, weight management, or wellness medicine.
• Familiarity with specialty laboratory interpretation.
• Experience in integrative or preventative medicine settings.
